My Takeaways from the New York ALCA Conference
Talking about the inevitable is never easy. But this week’s ALCA (Aging Life Care Association) Conference reminded me how important it is to help families move from denial to dialogue when it comes to end-of-life planning.
One of the most powerful sessions focused on Medical Aid in Dying. Many people don’t realize what choices exist, or that these options are still not legal in most states. A fellow ALCA member shared the story of her brother, who made the difficult, but intentional, decision to relocate to a state where Medical Aid in Dying was permitted. He planned not just the timing of his death, but the moments leading up to it: dinners with friends, celebrations with family, a final day at the beach with his wife and children, and heartfelt letters to those he loved most. It was a deeply human reminder of what dignity and agency can look like at the end of life.
Another theme that came up again and again: Hospice is often brought in far too late. Families typically engage hospice during the last 48–72 hours, when in reality many individuals could have benefited from 3–6 months of their support. Hospice isn’t just for the final moments, it’s a resource that brings comfort, structure, and guidance for both the individual and their loved ones. Even a simple hospice evaluation can be an important first step.
We also explored the growing role of Death Doulas, professionals who help guide individuals and families through the dying process. They ask the questions most people don’t even think about:
• Who do you want at your side?
• Do you want to hold someone’s hand?
• Who do you want to visit or call, you in your last days?
These conversations matter, and they ensure a person’s final weeks align with their wishes.
Beyond the sessions, it was wonderful connecting with so many fellow ALCA members. We shared best practices, learned from one another, and introduced our new division, Live Well Options. The support and camaraderie reminded me how strong this community truly is, I definitely felt the love.
