Summer McLaughlan, MSW, CMC, fosters strong collaborative relationships with her clients through consistent and clear communication, support and empathy, advocacy and ongoing education. Summer is passionate about health and wellness and believes that each domain of an individual’s life- the physical, emotional, mental, and spiritual – are all interconnected and equally important. Thus, Summer strives to focus on all aspects of a client’s life.

Summer received her Bachelor’s degree in Psychology and Sociology, and her Master’s degree in Social Work from Rutgers University. Summer has worked as a care manager for the past two years in the non profit human services sector, working closely with seniors and their families to provide ongoing assessment, advocacy, coordination with care providers, and referrals to appropriate services and resources in order to promote safety, independence, and overall well-being.

 

Prior to this, she developed extensive knowledge in the medical field given her experience working at Kessler Institute for Rehabilitation in West Orange, NJ and JFK Medical Center in Atlantis, FL. She has worked with a large variety of populations afflicted with illnesses and conditions including stroke, traumatic brain injury, amputations, joint replacements, orthopedic injury, and kidney failure. She has a strong understanding of rehabilitation modalities including physical, speech, and occupational therapies. As a discharge planner, she collaborated with her patients and families to coordinate safe and comprehensive discharge plans that considered medical, functional, financial, social, emotional, and cognitive factors.
