Lori Gephart

Gephart is a Registered Nurse with over 30 years of experience specializing in care for individuals with disabilities and chronic care management.

She earned her Bachelor of Science in Nursing (BSN) from Georgetown University and her Master of Science in Nursing (MSN) from Seton Hall University. She began her career in New York, working at Columbia Presbyterian Hospital (NewYork-Presbyterian) and Montefiore Medical Center. In 1994, Lori relocated to South Florida with her family, where she began a new chapter as a Medical Case Manager for the State of Florida.

Throughout her career, Lori has developed a mastery in care coordination, home health services, emergency management, long-term care, and home- and community-based services.

Lori is a dedicated advocate for client-centered care, committed to balancing independence with accessibility and support. She approaches every situation with empathy and compassion, helping individuals navigate transitions, overcome challenges, and improve their overall quality of life.
